New 2009 Electronic Health and Medical Forms

These new forms replace all old medical forms - class 1, 2 and 3. There is only one "type" now. This information and history parts of this form can be completed on your computer and then taken to a physician for the rest to be completed.

Below are links to important information for Eagle Scout rank advancement put together by our Razorback District Eagle Advancement Chair and the Council office. These documents are specific to Razorback District as they direct correspondence to our local Chair and not the Council office.  Scouts from other districts should obtain their documents elsewhere.

PRESS RELEASE NOTE:
Upon completion of your Board of Review and Eagle Court of Honor, please create a press release along with a head and sholders portrait and submit it to your local newspapers. Download the Eagle Press Release Information for writing instructions and email addresses to which to send them HERE

  This is a link to the form in PDF format on the NESA (National Eagle Scout Association) website. This is an EDITABLE PDF form that MUST be completed by using a computer. Handwritten forms will NOT be accepted by Razorback District. YOU MUST FIRST SAVE IT TO YOUR COMPUTER, THEN COMPLETE.
• The National Eagle Scout Scholarship application
  This is a link to the form in EDITABLE PDF format on the NESA (National Eagle Scout Association) website. We highly reccommend completing this form on a computer rather than by hand.
